New Copaxone log book

I was just looking at the 2015 hard copy log book for Copaxone, and it does not include the semi-annual health care review. I am very disappointed! I really found those helpful in really tracking my progress.

Does anyone have suggestions? I guess I could photocopy them from this year's log book.
