    Should I switch to Ocrevus?

    I failed Tecfidera and Plegridy.

    My MS specialist says my MRI is stable on Gilenya which I have been on for 15 months, but I can switch if I want and have already passed all the prerequisite blood tests.

    The reason I considered switching is my spasticity continues to progress to the point we are going to be doing a baclofen pump trial shortly. I have had new bladder issues. I have had 2 falls in the last 3 months.

    The MS symptoms are getting worse while I'm being told I'm stable - and don't get me started on fatigue. Provigil / Nuvigil give me headaches and nothing else could get approved except she JUST got amantadine through, so I'll see if that helps.

    Is it worth it to change meds to try and make my life more tolerable even if I'm on a "brain stable" DMT already?

    I was on Gilenya and switched due to new spinal lesion. They work via different mechanisms. The G goes after the T cells and the Ocrevus hits a type of B cell. Read up on how they work and then you will have more info to decide. There was a recent paper that did a meta comparison of the DMDs. Ocrevus "outscored" Gilenya due to being efficacious and having a lower serious side effect profile. This report is called the ICER report and though long, is worth reading or at least studying the graphs. I had to wait a while for the white count to rebound to normal- about 4 months.
