Continuous COBRA coverage and Medicare

Hello,

I am researching my medical coverage for when I do file for SSDI. I am currently working and have STD and LTD coverage.

Two questions:
1 - What are the specifics that allow up to 36 months of COBRA coverage? And is it state specific?
2 - If approved for SSDI does Medicare automatically kick in after 24 months of the date the disability was approved?

I was approved for LTD Aug 2015 then SSDI in Dec 2015 retroactive to my original disability date of Aug 2015.

I continued on my employer's medical plan since it was still available to me, but at 24 months when I'm eligible for
Medicare my company is "pushing" me over to that but assisting with the transition. I was told that 3 months before my eligibility date I would start receiving info and sure enough it has started to come in. FYI I'm just under 60 years old.
