Recent dr visits on updated disability report form

I have an updated disability report form I forgot my recent doctor visits from 2 years ago please help me I need responses from people on what I should do thanks linda nissen

Linda- call your doctor! they will have records on your visits!

Linda - if you have no record of your doctors at all and can't guess who you may have seen, call your insurer and get a list of claims. From the claims, you can get the doctor/office name, along with the date you visited.
