    Questions for people who are on Ocrevus

    I will be starting Ocrevus in the near future. I know it will suppress your immune system,
    were you instructed to avoid anything post infusion such as traveling, crowds etc....

    Did you have any lab tests or any other tests to do post infusion?

    Any other information / tips you could provide would be appreciated.

    No post labs required. I was in hospital for 2 weeks with sepsis [MS+infection+slightly suppressed immune system = trouble] and was instructed to wash hands frequently w/ soap & water before restarting ocrevus.

      No special instructions pre or post infusions.
      Knowing the upper respiratory infection risk, I avoid sick people and wash my hands before eating and after coming home from being elsewhere. Have now been on it for the equivalent of 4 full doses- no relapses, no new lesions.

        Ocrevus summary

        I've had 3 Ocrevus infusions. The two initial ones a week or two apart, and then the next full infusion 6 months later. I'm a couple of months away from my next one. My neurologist gave me each infusion with some IV benedryl and some IV prednisone. No reactions, no problems, just a day of extra energy from the prednisone.

        I received no special instructions. I've seen generic advice from this MS specialist/neurologist about being cautious about flu shots when on DMTs, but nothing else. I have noticed that things like cuts on my hands heal slower, and common sense tells me to avoid colds and things, but I haven't had any issues.

          Yes, definitely, you will want comprehensive blood work done down the road to see what is happening with important blood values. Your doctor may order blood work after your third treatment, so after 78 weeks and before 96 weeks.

          Decreased Immunoglobulins

          "OCREVUS decreased total immunoglobulins with the greatest decline seen in IgM levels. In MS clinical trials, there was no apparent association between immunoglobulin decrease and risk for serious infections.

          In the active-controlled (RMS) trials (Study 1 and Study 2), the proportion of patients at baseline reporting IgG, IgA, and IgM below the lower limit of normal (LLN) in OCREVUS-treated patients was 0.5%, 1.5%, and 0.1%, respectively. Following treatment, the proportion of OCREVUS-treated patients reporting IgG, IgA, and IgM below the LLN at 96 weeks was 1.5%, 2.4%, and 16.5%, respectively.

          In the placebo-controlled (PPMS) trial (Study 3), the proportion of patients at baseline reporting IgG, IgA, and IgM below the LLN in OCREVUS-treated patients was 0.0%, 0.2%, and 0.2%, respectively. Following treatment, the proportion of OCREVUS-treated patients reporting IgG, IgA, and IgM below the LLN at 120 weeks was 1.1%, 0.5%, and 15.5%, respectively." End Quote

          So, about 15 to 16% of those on Ocrevus experience immunoglobulin decrease to below the lower limit of normal (LLN). The significance of that is different for each individual and would be a topic that your neuro would address specifically pertaining to you.

            Hi cavan,

            I will be taking my 3rd full dose this month. I was not given any post instructions. I do wash my hands frequently and try to stay clear of sick people. I also get a flu shot once a year.

              Hi cavan,


              I traveled to Asia/India 3 months after my first infusions.

              I then had my flu shot (no live virus) as every year.

              I do wash my hands frequently, though I always have.

              If you have any concerns though I’d check with your neuro’s office.
