    What do they mean, deep white matter?

    Ok, I apoligize in advance for the lenght of this post. MRI of Brain shows numerous small suncentimeter foci of hyperintense T2 signal in subcortical white matter, deep white matter, left perventricular white matter, head of the caudate nucleus, one or 2 in subcortical U-fibers and a oval 6mm x 4mm nonspecific hyperintense signal in right external capsule.. MRI of cervical shows selling hyperdense signal (suspected MS lesion) at C2 ending at C3-4 disc. Also found a syrinx starting at C4 and ending at C7.

    When they talk about where they found the foci in the brian, what are they talking about? What is the deep white matter, what is the subcortical U fibers? and what is the external capsule?

    OK: So white matter and gray matter have different jobs in the brain.

    You seem to have numerous foci (meaning spots) in the subcortical area of the white matter. This is a common spot for MS lesions. This is not deep white matter.

    The deep white matter refers to the area a little closer to the grey matter of the brain. Also a spot for MS lesions.

    You also have periventricular spots a favorite spot for MS lesions to camp out. This just means it is situated beside or around a ventricle in your brain.

    The head of the cuadate nucleus is located in the basil ganglia and is responsible for memory and learning.

    The subcortical U-fibers are interesting as they are not as common in MS. They are myelinated fibers just at the junction of the gray matter and the white matter which travel in a tangential, rather than radial, fashion connecting areas of cortex to other areas of cortex.

    These are the slowest growing myelinating fibers in the CNS are begin at the embryonic stage.

    Your hyperintense nonspecific signal in the signal in right external capsule is in the brainstem.

    So you have a possible MS lesion at C2-C4.

    A syrinx (fluid filled cavity-rare) within the spinal cavity at C4-C7.

    I suppose you are having multiple symptoms. What are the neurologists telling you?

    I am sorry you are feeling bad...it is hard to wait.

      I would like to amend my post. U-fibers are seen in MS. I dont know what you are diagnosed with if anything.

      It is best to wait and get a thorough evaluation by your MS specialist for diagnosis. These are just suggestions about general locations.

      Several other diseases can also mimic these lesions. Was this your first MRI?

        Thank you for your reply. And thank you for your empathy. It has been a over-whelming last couple of weeks. My neurologist says I have MS. He did a lumbar tap and hopefully the results will be in soon. And the Neur Surgeon wants to do surgery on my back, two levels, disk issues. The hardest thing is not knowing exactly where the pain is coming from. The pain (burning, tingling and feels like someone socked me in the arm pain) is it the MS, is it the sprinx, or if the pain is lower, (spasms, tingling, numbness) it could be from the disk or the MS.

        And now I am having pain in the face, starts in between the eyes and goes down the side of my nose. And what is really scary, sometimes I will be in the shower and will accidentally inhale water. Two months ago I would have described myself as having very good health. rarely saw the doctor, very active and then wham, it hit me. The doc says I have had MS for a while, (the lesion on my cervical is "burnt" out. I guess now, the doc trying to figure out what type of MS do I have. Could I have those syptoms for years and thought it was something else?

          Facial issues (tingling/pain etc) are most likely the MS. The rest will have to sort itself out after surgery because it could be either one or both. If the sensations never go away, it is most likely not the MS, you should get a little relief or at least some lower intensity at times.

          It is complicated. Both could contribute. Wait for your LP to come back and go from there. It comes back pretty quick, so no worries there.
